How do I decide which service I need?

There are a lot of service options on offer at Succeed. It’s understandable that some carers aren’t sure where to start.

Through a 30 minute Discovery Interview (15 minute meeting and 15 minutes for documentation) our Allied Health Assistant we will explore your needs, and advise you about the club options we have available. This is a great chance for you to ask any questions you may have. It gives you and the team member the opportunity to work out if Succeed is the right fit for you.

At that stage, the team member may suggest some possible services to consider more closely to begin with.

If we are the right fit for your young person’s needs, we will book a one hour appointment for a needs assessment where we will generate a Youth Top Problems List. We will then ask you which problems are your first priority to address, and recommend a service plan based on that information.

Funding/payment options?

NDIS Services and Payment Terms

Succeed Healthcare Solutions is proud to be a registered NDIS provider, offering a range of services to support individuals under the National Disability Insurance Scheme. Our payment terms for NDIS services require payment on the day of the service. To ensure a secure and convenient payment process, we use Medipass/Tyro, a reliable credit card payment system, for processing payments on the day.

For our plan-managed clients, we understand that certain cases may require payment after the day of service. In such instances, we offer a line of credit/credit loan to clients for the provided service. We work with several preferred plan managers who pay within 2-3 days, including Sunshine Coast Plan Manager, iplan, Caring for Carers (subject to client pre-approval of invoices and awareness from the Plan Manager), My Plan Support, Plan M8 Australia (require client pre-approval of invoices), Bright Plan Management, Plan Management People, Kera’s Plan Management and Square & Spiral.

Additionally, we are flexible during the transition period from other plan managers who may not adhere to our preferred payment terms. In these situations, we will accept payments from non-preferred plan managers for up to a month. We also make exceptions for non-preferred plan managers who agree to pay within 2-3 days.

To ensure a smooth process, we kindly request storing plan managed clients’ credit card details securely in Medipass/Tyro. This measure is taken as a precaution in the unlikely event that a plan manager does not settle an invoice within 2 weeks.

Please note that we no longer service NDIA managed clients due to concerns regarding claiming payments. For clients under NDIA management, we encourage considering self or plan management with a plan manager who offers prompt payment within 2-3 days.

Medicare Services

Our services also extend to Medicare rebatable occupational therapy, physiotherapy, and exercise physiology. This includes individual therapy sessions, either in person or via telehealth, provided the appointment meets Medicare’s minimum requirement of at least 20 minutes of contact time. Additionally, group sessions with six or more participants for occupational therapy are eligible for Medicare rebates.

It is important to obtain a Chronic Disease Management Plan or a Mental Health Care Plan referral from your GP before commencing Medicare rebatable services with us. Please be aware that a gap fee applies.

Private Health Funds

We offer occupational therapy, physiotherapy, and exercise physiology services that are claimable through select private health funds. These claims can be made for both individual and group sessions, whether they are conducted in person or via telehealth. To ensure coverage, we recommend checking with your health fund about our services.

Private Paying Clients

In addition to NDIS, Medicare, and private health fund clients, we also accept private paying clients. Our payment terms for private services require payment on the day of service or invoice. As we conduct our services off-site, often at exciting venues, people’s homes, or childcare centres, we utilise the secure credit card payment system, Medipass/Tyro, for processing payments on our clients’ behalf. Please rest assured that we are unable to access or view client credit card details through this system, ensuring the utmost privacy and security for our clients’ transactions.

At Succeed Healthcare Solutions, we adopt a person-centred approach to support planning. We believe that each participant in the National Disability Insurance Scheme (NDIS) is unique, and their support plan should reflect their individual needs, preferences, strengths, and goals. Our goal is to actively involve the participant in the development of their support plan, ensuring it is tailored to their specific requirements.

2. How is the support planning process conducted?

Our support planning process begins with a comprehensive and effective assessment, conducted in collaboration with the participant. We engage with pertinent individuals in the participant’s support network, such as family members, allied health professionals, and carers, to gather relevant information for thorough planning.

3. What does the assessment include?

During the assessment, we identify any potential risks and associated management strategies, which are documented in the support plan. We also identify and record the participant’s strengths to ensure a holistic approach to their care.

4. How often are support plans reviewed?

Support plans are generally reviewed quarterly for clients who attend clubs only based on risks and goals. The review process involves collaboration with the participant and their trusted decision makers (TDM). During the review, we update the support plan as needed, and with consent, communicate changes to relevant stakeholders. The Therapy Assistant Plan for clients who access Allied Health Assistant services is reviewed as needed depending on the progress towards goals ie quarterly. 

5. How do you ensure participants’ involvement in the process?

We prioritise effective communication with the participant throughout the support planning process. Their involvement is fundamental in decision-making and goal-setting, allowing us to create a plan that aligns with their desires and aspirations.

6. Do you adhere to NDIS Practice Standards?

Yes, at Succeed Healthcare Solutions, we strictly adhere to the NDIS Practice Standards, particularly the Support Planning Standard outlined in the NDIS Practice Standards: Core Module – 3 Provision of Supports Standard. We aim to provide the highest quality of service delivery by meeting the standards set forth by the NDIS Commission.

  2. Review and Progress: We continuously monitor each young person’s progress during their time in our clubs. After an initial 24 sessions (2 sets of 12 sessions), we review their achievements, challenges, and emerging needs. Based on this evaluation, we may recommend transitioning to a different program within our range of services to target specific areas of development.
